Do not submit PHI
The website intake is not intended for patient records or protected health information. Do not submit patient names, dates of birth, member or policy numbers, claim images, clinical records, diagnoses, treatment information or other patient-identifying information through the standard website form or ordinary email.
If information requiring a secure exchange is needed for an engagement, 1st Source will provide an appropriate process after the scope of work is established.
How inquiry information is used
Information submitted through the website may be used to review your request, determine whether 1st Source may be able to assist, respond to you, prepare an engagement discussion, maintain business records, protect the website and improve business operations.
Website inquiry submissions are sent by email and are not intentionally stored as form records in the WordPress website database.
